Did you know the meaning of SF10 based on the DepEd? It is important to know the meaning of every document you need in school. The School Form 10 (SF10) is a summary of your grades from Grade 1 to your current level. Thus, it is also called Permanent School Record, which is used in all official academic transactions. This form is a mandatory requirement in your enrollment, learner recognition, and other related documentation. Also, schools consider the SF10 an official document of the learner, which they may use for certain legal purposes. DepEd revised the form to include only the necessary and permanent data elements. The data is grouped into two main categories: student personal information and student academic progress and achievement across the elementary grades.
List the name of the school and its school identifier in each table for every grade level. Tabulate all learning areas from the Matatag K-10 curriculum and sort them in the same order. The SF10-ES provides more than six (6) tables for reporting. In cases when students are no longer in school or remain in a particular grade. Additional pages can be prepared if necessary.
Meaning of DepEd SF10 and Other Important Details
The DepEd School Form 10 (SF10) is also known as the School Permanent Record. It contains all the academic records of a learner. It serves as a summary of all the report cards and is compiled into a single official document. This document is permanent, meaning all academic achievements and failures are recorded and preserved. It shows the highest and lowest grades a learner received across all grade levels attended. Additionally, the SF10 functions like a timeline of your academic journey. It reflects when and where you studied, any transfers, and if applicable, any instances of failure.
Fill in the student’s personal information at the top of the form. To maintain uniformity, print only the official DepEd logo and seal on the SF10-ES. Place the school’s dry seal in the certification field once the student meets the requirements in Grade 6. At the end of a learner’s journey in Elementary Education, a certification must be completed at the bottom of the SF10. This section indicates whether the learner is eligible to advance to the next grade level. It must also include the name and school ID of the institution where the learner completed their studies.
How to prepare SF10 (Form 137)?
The Grade 1 teacher will prepare only one (1) copy of the SF10-ES and attach appropriate supporting documents (e.g., a copy of a birth certificate, kindergarten certificate/Early Childhood Care and Development (ECCD) Checklist, progress report, or another equivalent document). If the student continues at the same elementary school (ES), the SF10-ES will be transferred to the Grade Level teacher. Follow the same procedure in each subsequent grade until the student completes Grade 6.
If the learner transfers to continue at an entry-level position at another elementary school, the receiving school must request the original copy of the SF10-ES and other relevant documents. The sending ES, upon receipt of a written request or system notification of transmission through the LIS, whichever occurs first, will create one certified photocopy of the SF10-ES for its file and send the original SF10-ES along with the other relevant documents to the ES recipient. The ES recipient will update the LIS upon receipt of the documents and continue to complete the original SF1O. Title V Guidelines for Students’ School Records and Title VI Other Provisions Provided in DO 54, s. 2016 strictly follow.
If the student has transferred to JHS, the ES, upon receipt of a written request or system notification of transfer through LIS, will create one certified copy and send it to the requesting JHS. The JHS recipient through the 7th grade advisor should use the certified copy of the SF10-ES as a reference to create the SF10-JHS.
Important Reminders in preparing DepEd SF10 (Learner Permanent Record)
- Any erasures or changes to the document must be verified and signed by the head of school in ink.
- Print the SF10-ES on plain, legal-sized paper. Avoid commercial printing that may lead to unnecessary costs.
- Prepare the SF10-ES at the end of the school year or when students transfer during the school year.
- To reiterate the provisions of DO8, s. 2015 entitled Policy Guidelines on Classroom Assessment for the K to 12 Basic Education Program and DO47, s. 2016 titled Common Policy on Kindergarten Education, students enrolled in a kindergarten program must use the ECCD Checklist instead of Form 137 or SF10-ES.
