The School Form 9 (SF9) or Report Card is the official document used to record and report the learner’s academic performance and behavior throughout the year. It describes a student’s academic progress in numerical grading from 60 to 100. In addition, schools often give the report card to the student or the student’s parents once to four times each year. The standard report card uses a grading system prescribed in DepEd Order 29 series 2017. On this page, you can find the official template of the form. Importance of SF10 or School Report Card

The Department of Education (DepEd) issued DO 11, s. 2018 – Guidelines on the Preparation and Checking of School Forms – to guide the preparation, updating, and evaluation of school forms in basic education. One of the required forms during this process is the Report Card, which also serves as the basis for data entry in the School Permanent Record. However, the main purpose of School Form 9 (SF9) is to report the learner’s progress to parents or guardians. The form alone is sufficient to reflect each student’s academic achievements or areas of concern. Thus, the teachers can communicate with parents about their child’s needs and recommend appropriate interventions.

Reminders for Using SF9 or Progress Report Card

Please take note of the following important guidelines in preparing the SF9 or Progress Report Card for all Grade levels except Kindergarten:

Rename the Form from Form 138 to SF 9 Progress Report Card (DO 58, 2017);
The content and format as prescribed by DO 8, 2015 (DO 58, 2017); and,
The learner’s name should be written the way it was generated in the SF5 (from LIS). (DO11, 2018)
