Purpose of CSC Form 212 Personal Data Sheet PDS 2025
The Personal Data Sheet is used to collect, store and organise details, including personal, professional, and skill-related information. All public employees or applicants need to submit this requirement. The policy aims to require all public officers and employees to declare and submit each year a true, detailed, and sworn Statement of Assets, Liabilities, and Net Worth (SALN). This includes the disclosure of their business interests, financial connections, and, to the best of their knowledge, their relatives who are in government service. It also ensures that the declarant properly reports the assets, liabilities, net worth, financial connections, and business interests of their spouse and unmarried children below eighteen (18) years old living in the household.

Important Points in Filling Out Personal Data Sheet PDS or CSC Form 212
- a. To prevent unauthorised insertions or removal of pages, pagination should follow the format “page 1 of _ pages,” “page 2 of _ pages,” and so on.
- b. When attaching additional documents, the applicant must sign the spaces provided below the certification to authenticate all pages.
- c. If applicable, the applicant must accurately complete all required information about their spouse or family members as indicated in the PDS.
- d. When the applicant cannot provide certain family information, they must attach a brief written explanation to the PDS.
- e. Heads of agencies may designate personnel authorised to administer oaths for PDS submissions that require sworn statements. The delegation must be made in writing.
- f. “Head of agency” includes the head of the main office and/or regional offices responsible for accepting the PDS.
- g. The applicant must fill out all applicable fields in the PDS. They must mark items that do not apply with “N/A” or “Not Applicable.”


- h. The applicant may complete the form by hand, computer, or typewriter, as long as they provide their original signature. They must write clearly and legibly if completing the form by hand.
- i. The applicant may use additional sheets if necessary. They must indicate their name, position, agency, and the year covered on each sheet and sign every page.
- j. The applicant must avoid unnecessary markings, doodles, or alterations on the form.
