Step 1. Gather the required documents
To update your registration details, you will need to provide a valid ID and proof of address. Acceptable forms of ID include a passport, driver’s license, or any government-issued ID.
Note: You have to bring Affidavit of Lost are one of the important requirements in this process.
Step 2. Visit your ISP Store or Customer Service
You can go to any authorized SMART, GLOBE or DITO Store or Customer Service depending on your particular provider to update your registration details.
Step 3. Fill out the required Form
You will be asked to fill out a SIM Replacement Form. Make sure to provide your correct details and sign the form.
Step 4. Submit the form and required documents
Once you have completed the form and gathered the required documents, you can submit them to the SIM Card retailer or dealer.
It is also important to note that if you lose your sim card, you need to report it to the customer service hotline and report it as lost. They will deactivate the SIM card and provide you with a new one, this is also a way of preventing illegal activities. - Smart: You can reach Smart’s customer service hotline by dialing *888 from a Smart, TNT or Sun mobile phone or (02) 8 888 from a landline.
- DITO: You can reach DITO’s customer service hotline by dialing *177 from a DITO mobile phone. They also have numbers for different concerns, you can check on their website or by calling the customer service hotline number mentioned.
