The Department of Education (DepEd) announces the enrollment period as June 9–13, 2025, in all public schools. The department expects that the enrollment population will exceed 21 million learners. The usual documentary requirements for enrollment are a Report Card and a Birth Certificate, except for the Enrollment Form and Confirmation Slip, which vary depending on the grade level you are enrolling in. The official copy of the DepEd Confirmation Slip is uploaded here for both public and private schools. For a more detailed list of requirements, please refer to our Enrollment Information section.
What is the DepEd Confirmation Slip?
Learners in Grades 2 to 6, 8 to 10, and Grade 12, or their parents, must submit the DepEd Confirmation Slip to confirm enrollment in the same school as the previous year. They must also submit the Report Card for these grade levels. Essentially, the Confirmation Slip confirms a learner’s continued enrollment in their current school. Meanwhile, all public elementary and secondary schools require the Basic Education Enrollment Form for incoming Kindergarten, Grades 1, 7, and 11 enrollees.
